Is the Effective Quadriceps Moment Arm Influenced by Patella Alta (a Dynamic in Vivo Study)?
نویسندگان
چکیده
INTRODUCTION The moment arm (ma) of a tendon is a crucial quantity for understanding musculoskeletal dynamics as it defines how the linear force within a muscle is transformed into a torque. For most tendons this is a fairly well established parameter as it can be defined from the kinematics of the tendons’ origins, insertions, and/or wrapping points. Yet, for the quadriceps this relationship is complicated by the fact that the patella serves as an intermediary (a dynamic fulcrum) between the quadriceps and tibia. Thus, the transfer of the quadriceps force into a torque on the tibia cannot be directly calculated. Therefore, the term effective quadriceps moment arm (EQma) was coined to define the ability of the quadriceps to generate a torque on the tibia [1]. To date, no study has quantified this parameter during dynamic in vivo movement. Furthermore, the critical question of how patellofemoral (PF) kinematics may influence the EQma remains unresolved. Specifically, patella alta has been widely implicated as the source of a reduced EQma in patients with cerebral palsy, contributing to an already weakened extensor mechanism [2,3]. In contrast, a recent study demonstrated that individuals with patella alta had a significantly larger EQma [4]. Therefore the purpose of this study was to determine, using in vivo dynamic MRI, the EQma during a knee extension exercise in a large group of asymptomatic controls. A secondary goal of the study was to determine if the EQma was correlated with PF or tibiofemoral (TF) kinematics.

متن کاملInfluence of patella alta on knee extensor mechanics.
The purpose of this study was to compare the knee extensor mechanics in persons with and without patella alta. Thirteen subjects with patella alta and 14 subjects with normal patellar position participated in the study. Sagittal and axial MR images of the knee were acquired at 0 degrees , 20 degrees , 40 degrees , and 60 degrees of knee flexion. Measurements of actual moment arm, patellar ligam...
متن کاملDynamic In Vivo Moment Arms of the Individual Quadriceps Components
INTRODUCTION: Patellofemoral pain (PFP) is closely related to static patellar malalignment and patellar maltracking. The etiology of patellar maltracking is multifactorial; relating to femoral shape, passive soft-tissue restraints; and/or an imbalance of forces on the patella. A number of structures, both passive and active, contribute to the force balance on the patella. Previous studies have ...
متن کاملInfluence of patellar position on the knee extensor mechanism in normal and crouched walking.
Patella alta is common in cerebral palsy, especially in patients with crouch gait. Correction of patella alta has been advocated in the treatment of crouch, however the appropriate degree of correction and the implications for knee extensor function remain unclear. Therefore, the goal of this study was to assess the impact of patellar position on quadriceps and patellar tendon forces during nor...
متن کاملQuantification of Dynamic In Vivo 3-D Muscle Moment Arms Using Cine-PC MRI
INTRODUCTION A number of structures, both passive and active, contribute to the force balance on the patella. To understand the relative contribution of each quadriceps component to the balance of forces on the patella, the individual moment arms of the quadriceps tendons must be known. Previous studies have investigated the moment arm of the patellar tendon and the quadriceps muscle as a whole...
